What We Collect
When you fill out a form on this site, we collect what you type in: your name, phone number, email address, and anything you write in the message field. That is it. We do not ask for, and you should not send, detailed health or insurance information through this website.
How We Use It
We use your information to answer your question, schedule your visit, and follow up about your care with us. We may contact you by phone, text, or email using the details you gave us. We never sell your information, and we never share it with anyone outside the practice and the tools we use to run it.
The Tools We Use
- Online scheduling. Booking an appointment happens through our scheduling partner, Flexbook, under its own privacy policy.
- Financing. If you apply for financing, you apply directly with Cherry or CareCredit on their sites, under their own policies. We never see your application details.
- Website hosting. Like nearly every website, our host keeps standard technical logs (such as IP addresses and pages visited) to keep the site running and secure.
- Embedded video. Some pages include YouTube videos, which may set cookies if you play them.
- Advertising measurement. We may use measurement tools from platforms such as Google and Meta to understand how people find us. These tools do not receive your name or health information from us.
Your Health Information
Once you become a patient, your health information is protected under HIPAA and handled according to our Notice of Privacy Practices, which is available at our office. This website policy covers only what you share through the site itself.
Your Choices
You can ask us at any time to see the information we have from your website submissions, correct it, or delete it. Just call or email us using the details below. You can also opt out of follow-up messages whenever you like.
Children
This site is meant for adults. We do not knowingly collect information from children under 13 through this website.
